What is a personal care plan?

A personal care plan is a written, individualized document that outlines a resident's specific care services, daily support needs, and health requirements, reviewed and updated as their circumstances change.

A personal care plan is a written document tailored to each resident that spells out their specific care services, daily living support, and medical needs. It functions as the central record of what a resident requires and how staff will address those needs on an ongoing basis.

The plan typically covers areas such as bathing and grooming assistance, medication management, mobility support, meal preparation, and any specialized care related to chronic conditions or mobility limitations. It documents preferences the resident has expressed about how and when they want to receive care, as well as emergency contacts and any specific instructions from their physician.

Plans are not static. An effective personal care plan is reviewed regularly, often quarterly or when there is a significant change in the resident's health status, functional ability, or living situation. This keeps the plan aligned with current needs rather than relying on outdated information. If a resident develops new medical conditions, experiences a fall, or requires additional support, the plan gets updated to reflect that shift.

For residents and their families, the personal care plan provides transparency about what services are included and helps ensure consistent care delivery. For assisted living facilities, it serves as the operational guide that helps staff provide coordinated, individualized care. Having a clear written plan reduces confusion, supports continuity when staff change, and documents the care agreement between the facility and the resident.
